Support Coordination

Our Support Coordination service is designed to be your guiding compass through the National Disability Insurance Scheme. We navigate the complexities, helping you to make the most out of your plan, access the right services and make informed choices about your life. Once we understand your priorities for your physical, social and emotional wellbeing, then we can look at how we can work together to best meet your needs.

Level 2:

Coordination of Supports

It is our role to help you to understand your NDIS Plan. We aim to be your coach and supporter. We can help to connect you with community support, organise and coordinate new services, evaluate how your supports are working, and make any changes needed along the way. We can also help you to prepare for the review of your NDIS Plan to make sure that future plans meet your needs.

Here are some examples of what a Support Coordinator will likely do:

  • Help you to understand your NDIS Plan

  • Work with you to connect with providers and establish services

  • Work with you to maintain and monitor your services

  • Provide targeted Support Coordination, e.g. by helping you to find a new place to live

  • Support you to access new Assistive Technology (equipment)

  • Build your capacity to navigate the NDIS more independently

  • Crisis planning, prevention, mitigation and action

  • Report to the NDIA as required

Level 3:

Specialist Support Coordination

Our Specialist Support Coordination service is designed to address complex barriers that may impact your ability to access appropriate support. We work with you, your providers, your family and other relevant systems to develop innovative solutions to overcome these barriers. We may develop a complex service plan tailored to you to ensure that your entire support system understands how to work together effectively and to ensure your NDIS Plan can be fully implemented.

Here are some examples of what a Specialist Support Coordinator will likely do:

  • Help you to connect with supports and services

  • Help you and those around you to address complex barriers

  • Provide targeted Support Coordination that is responsive to your immediate needs

  • Plan for and manage crises

  • Design a complex service plan to ensure that all of your providers and those around you know how to support you best

  • Build your capacity and resilience so you can navigate your life more independently

  • Report to the NDIA as required
